PUTTING HEALTH IN PERSPECTIVE: IRREVERSIBLE DISABILITY
Rene Dubos, biologist and philosopher, aptly summarized the thinking of his contemporaries by defining health as "a quality of life, involving social, emotional, mental, spiritual, and biological fitness on the part of the individual, which results from adaptations to the environment. The concept of adaptability, or the ability to successfully cope with life's ups and downs, became a key element of the overall health definition. Eventually the term wellness became popular and not only included the previously mentioned elements, but also implied that there were levels of health in each category. To achieve high-level wellness, a person would move progressively higher on a continuum of positive health indicators. Those who fail to achieve these levels may move to the illness side of the continuum. Today, the terms health and wellness are often used interchangeably to mean the dynamic, ever-changing process of trying to achieve one's individual potential in each of several interrelated dimensions.
Physical health
This dimension includes characteristics such as body size and shape, sensory acuity and responsiveness, susceptibility to disease and disorders, body functioning, physical fitness, and recuperative abilities. Newer definitions of physical health also include our ability to perform normal activities of daily living (ADLs), or those tasks that are necessary to normal existence in today's society. Being able to get out of bed in the morning, being able to bend over to tie your shoes, or other usual daily tasks are examples of ADLs.
Intellectual health
This dimension refers to the ability to think clearly, to reason objectively, to analyze critically, and to use "brain power" effectively to meet life's challenges. It means learning from successes and mistakes and making sound, responsible decisions that take into consideration all aspects of a situation.
Social health
This dimension refers to the ability to have satisfying interpersonal relationships: interactions with others, adaptation to various social situations, and daily behaviors.
Emotional health
This dimension refers to the feeling component - to express emotions when appropriate and to control emotions both when it is inappropriate to express them and to avoid expressing them in an inappropriate manner. Feelings of self-esteem, self-confidence, self-efficacy, trust, love, and many other emotional reactions and responses are all part of emotional health.
Environmental health
This dimension refers to an appreciation of the external environment and the role individuals play in preserving, protecting, and improving environmental conditions.
Spiritual health
This dimension may involve a belief in a supreme being or a specified way of living prescribed by a particular religion. Spiritual health also includes the feeling of unity with the environment - a feeling of oneness with others and with nature - and a guiding sense of meaning or value in life. It also may include the ability to understand and express one's purpose in life; to feel a part of a greater spectrum of existence; to experience love, joy, pain, sorrow, peace, contentment, and wonder over life's experiences; and to care about and respect all living things.
A well individual might display the following characteristics:
- A realistic sense of self, including personal capabilities and limitations
- An appreciation of all living things, no matter how ugly or beautiful, how unique or different, or how great or small
- A willingness to understand imperfection, to forgive others' mistakes, and to grow from personal mistakes or shortcomings
- The ability to laugh, to cry, and to genuinely "feel" emotions without getting lost in emotional upsets
- The ability to function at a reasonable level physiologically
- The ability to maintain and support healthy relationships with family, friends, intimate partners, and strangers
- An appreciation for one's role in preserving and protecting the environment
- A sense of satisfaction with life and an appreciation for the stages of the life experience
- A zest for living, coupled with a curiosity about what each new encounter, each new day will bring
- A respect for self, as well as a respect for others
- A realistic perspective about life's challenges and the skills to cope with life's stresses and challenges
- A balance in all things
Many people believe that wellness can best be achieved by the adoption of a holistic approach, in which a person emphasizes the integration of and balance among mind, body, and spirit. Persons on the illness and disability end of the continuum may have failed in one or more of these areas. This does not mean, however, that someone who is physically disabled can never achieve wellness. Rather, achieving wellness means attaining the optimum level of wellness for a given person's unique set of limitations and strengths. A physically disabled person may be functioning at his or her optimum level of performance; have satisfying interpersonal relationships; work to maintain emotional, spiritual, and intellectual health; and have a strong interest in environmental concerns. In contrast, those who spend hours lifting weights to perfect the size and shape of each muscle but pay little attention to nutrition may look healthy but may not maintain a healthy balance in all areas of health. Although we often consider physical attractiveness and other external trappings in measuring the overall health of a person, appearance and physical performance indicators are actually only two signs of physical health, indicating little about the other dimensions.
