Major Uses: Hydrochlorothiazide is used to lower blood pressure and reduce the build up of fluid that can occur in heart, liver and kidney failure. It can be effective in reducing fluid accumulation prior to menstrual periods. Sometimes, some types of kidney stones will respond to hydrochlorothiazide.
How it Works: Hydrochlorothiazide acts on the kidney to decrease the re-absorption of salts and water.
Usual Dosage: 25-50 mg daily in the mornings is the usual adult dose, although considerably higher doses can be used if necessary.
Common Side-Effects: Leg cramps can occur as may dizziness and tiredness. Prolonged use can lead to a depletion of potassium. Often a potassium replacement is used at the same time to prevent this potential problem. (Some of the tablets listed above have diuretics other than hydrochlorothiazide in them, which help to conserve potassium.)
Actions with other Drugs: Some anti-inflammatory drugs may reduce the effect of this drug. Digitalis side-effects are more likely due to potassium depletion. Lithium levels can be increased. Alcohol may accentuate the fall in blood pressure that occurs on standing. Other medications which lower blood pressure can cause excessive blood pressure falls.
Pregnancy, Breastfeeding: Diuretics are not generally used to treat raised blood pressure in pregnancy. It may accentuate problems of blood flow to the fetus.
Milk supply may diminish when it is used by breastfeeding mothers, so it is not commonly used.
Special Features, Comments: Blood tests may be necessary from time to time to check on potassium levels. Long-term treatment may impair glucose tolerance mimicking diabetes; though stopping the drug reverses this. An increase in uric acid level may occur and lead to gout. In severe liver disease this medication may be dangerous. Thiazide diuretics may increase blood cholesterol levels.
