In a study of 194 infants with major malformations, the frequency of first-trimester penicillin use was no greater than expected.
No teratogenic effect was observed among the offspring of rabbits treated during pregnancy with penicillin in doses similar to those used in humans.

Risk Related to Breast-feeding
Penicillin is excreted in the breast milk in very low concentrations. The amount of penicillin that the nursing infant would be expected to receive is less than 1% of the lowest weight-adjusted therapeutic dose, based on data from 11 lactating women.
