4. Action

Ropinirole reduces the severety of motion disorders on any disease state. It debilitates the symptoms and signs of the disease, reduces manifestation of motion disorders. It reduces the manifestation of depression and neuropsycologic disorders, including the highest brain functions: memory, attention, psychomotor coordination, speech, calculating, thinking, orientation, planning and control of higher mental performance.

5. Posology and Administration

Ropinirole should be taken orally 3 times a day. The starting dose on any stage of Parkinson’s disease should be 0,375 mg/daily. If needed the dose may be increased, but not oftener than every 5-7 days (up to the attainment of the optimal therapeutic effect with account of the appearance of side effects): the 2-nd week of treatment - 0,75 mg/a day, the 3-d week - 1,5 mg/a day and further on, increasing the dose by 0,75 mg a week up to the maximal one - 4,5 mg/daily. A supporting dose should be 1,5-4,5 mg/a day in 3 intakes. In event of combination with Levodopa - a Levodopa dose may be reduced.

In patients with kidneys failure a dose depends on creatine clearance: at Cl creatinine is more than 60 ml/min - 0,125 mg each 3 times a day (the maximal dose is 1,5 mg each 3 times a day), 35-59 ml/min - 0,125 mg each 2 times a day (maximum 1,5 mg each 2 times a day), 15-34 ml/min - 0,125 mg once a day (maximum - 1,5 mg once a day).

6. Counterindications

Hypersensitivity, arterial hypotension, collapse, myocardial infarction (acute period), pregnancy, breastfeeding.

7. Should Be Taken with Precautions, if…

Patients should be warned about the risk of hallucinations (especially in elderly people). Care should be taken in administration to patients with kidneys failure, in sufferers of very severe compromised kidneys functions. Ropinirole withdrawal is recommended to implement gradually (during one week). Drivers of vehicles and the people whose profession is connected with elevated attention concentration should not use Ropinirole during their work.

8. Interface with Other Medications

Symptoms: vomit.

10. Side Effects

On the part of nervous system and sense organs: fatigue feeling, drowsiness/insomnia, hallucinations, delirium, blocking, mental fog, dizziness, anxiety, depression, inflammatory processes of the cavity of mouth, gorge, gullet, throat, scarry strictures and edemas, dystonia, inducement to the action which is difficult to curb, thinking disorders, suicidal tune, swift rhythmic motions of the limbs or the trunk, low superficial cutaneous touch, sluggishness, insufficient motion activity, impulsive muscle spasm, imbalance in standing, loss of coordination, double vision, ophthalmic nerve paralysis, conjunctivitis, reduction of hearing in singular cases (in event of quick dose reduction or its abrupt withdrawal) – malignant neuroleptic syndrome (fits, muscle weakness, impairment of consciousness, headaches, dizziness, sense of pain and intermissions of the heart).
On the part of cardiovascular system and blood – significant drop of arterial tension at the moment of rising, tachycardia, arrhythmia.
On the part of respiratory system: pant, running nose, inflammation of paranasal sinuses, inflammation of the gorge, flu-like syndrome and cough enhancement.
On the part of gastro-intestine tract organs: nausea, vomit, abdominal swelling, diarrhea, dry mouth, anorexia, constipation.
On the part of locomotor system: muscular spasm, convulsion in the legs muscles, jerks, and weakness of muscles, arthritis, and purulent inflammation of juxta-articular of bursa eem.
Others: fever, peripheral edemas, perspiration, rise of intraocular tension, drop of libido, impotency, body mass reduction, polyuria, infections of urinary tract, pain syndrome, including chest pain, pain in the stomach, low back pain, neck pain, voice change, allergic reactions.

11. Pregnancy and Lactation

In the period of pregnancy Ropinirole therapy is adverse and only in cases of extreme necessity is possible. On the period of treatment breastfeeding should be withdrawn.

12. Driving

Drivers of vehicles and people whose profession is connected with elevated attention concentration should not use Ropinirole therapy, if there is no possibility to avoid such actions during the treatment.
