Norgestrel is a synthetic progestin that is used alone or in combination with an estrogenic compound as an oral contraceptive and in treatment of menstrual disorders. The usual dose is 75-150 mcg/d, but doses as large as 2000 mcg/d are given when norgestrel is used as a postcoital contraceptive.

Contraceptive doses of norgestrel are unlikely to pose a substantial teratogenic risk, but the data are insufficient to state that there is no risk.
A minimal risk for virilization of the genitalia in a female fetus may exist with maternal use of large doses of norgestrel during pregnancy. This assessment is based primarily on studies involving use of progestational hormones at doses much greater than those currently employed for conventional oral contraception or treatment of menstrual disorders.

No epidemiological studies of congenital anomalies in infants born to women who took norgestrel specifically during pregnancy have been reported. Extensive epidemiological data are available regarding the frequency of malformations in infants of women who took oral contraceptives early in pregnancy, but the findings are inconsistent. If the risk of congenital anomalies in children of women who took oral contraceptives early in pregnancy is increased, this increase is small compared to the background risk of birth defects in the general population.
Studies in mice and rabbits suggest that maternal use of conventional oral contraceptives containing norgestrel during pregnancy is unlikely to increase the children's risk of malformations greatly.

Risk Related to Breast-feeding
Norgestrel is excreted in the breast milk. The amount of norgestrel that the nursing infant would ingest is between Serum levels of norgestrel in three infants whose mothers used a norgestrel-only contraceptive while breast-feeding were between 0 - 38% of the lowest therapeutic serum level for oral contraception.
No adverse effects of norgestrel use were found on milk volume in lactating women or on the growth or psychomotor development of their infants.
Breast enlargement was described in one 18-month-old infant whose mother had been on norgestrel for three months. The enlargement disappeared six months after termination of breast-feeding. Nevertheless, gynecomastia has not been consistently observed in other reports.
