It is combined with a variety of progestagen hormones. (Ethinyloestradiol is an oestrogen.) It can also be used for hormone replacement at and after menopause, to control irregular or excessive menstrual bleeding and in a variety of other medical problems. How it Works: The contraceptive effect of this medication is due to its ability to prevent hormones (secreted from the pituitary gland at the base of the brain) from being released. As a result, the ovary is not stimulated and no ova are released. Oestrogens stimulate a wide variety of other cells in the female body, most importantly the lining of the uterus (the endometrium) and this stimulation is responsible for the other therapeutic effects of the drug. Usual Dosage: 10-20 micrograms per day for hormone replacement is the average dose for most women. A higher dose 30-50 micrograms is the range of dose used in combined oral contraceptive pills. Common Side-Effects: Nausea and vomiting are the most common side-effects of this drug. Breast tenderness and a gain in weight are common problems with combined oral contraceptives. Abdominal cramps, change to skin colour, a reduction (or occasional increase) in menstrual bleeding, bleeding in between cycles, increased frequency of migraine headaches, change in mood, decreased milk supply in breastfeeding women, altered liver function and a rise in blood pressure may occur. Actions with other Drugs: Some antibiotics can reduce the absorbed amount of oestrogen and decrease the contraceptive effect or cause breakthrough bleeding. The effect of blood pressure and blood glucose lowering drugs may be offset by ethinyloestradiol. Smoking increases the risk of some serious side-effects. The dose of oral anticoagulant drugs may need to be altered. Pregnancy, Breastfeeding: Combined oral contraceptive pills have not been shown to cause problems with pregnancy while they are being used, though they should be stopped before attempting to fall pregnant. The 'pill' decreases the supply of breast milk and is not recommended for use while breastfeeding. Special Features, Comments: Serious problems can occur with the use of oestrogens. These include blood clots affecting the brain, lungs and heart, gall bladder disease and possibly increase the risk of cancer of the uterus (if used alone as hormone replacement).
