Category: antidysrhythmic.
Indications: ventricular arrhythmias, especially after myocardial infarction.
Cautions: liver and kidney impairment, pregnancy, lactation, children.
Contraindications: second or third degree A-V block, cardiogenic shock, bradycardia.
Side effects: nausea, vomiting, tremors, blurred vision, lethargy, rash, fever, conduction defects, bradycardia, hypotension, atrial fibrillation, constipation, Incoordination, agranulocytosis, drowsiness, confusion, ataxia, convulsions, hepatic and psychiatric disorders.
Drug interactions: morphine, atropine, antacids reduce its oral absorption; phenytoin and rifampicin reduce its effect; increases theophylline toxicity; effect antagonized by hypokalemia due to diuretics.
Dosage forms: capsule 50,150 mg, injection 250 mg/10 ml.
Dose: 400 mg orally, followed after 2 hours by 200-250 mg 3-4 times daily; intravenous injection, 100-250 mg at a rate of 25 mg/minute with ECG monitoring followed by infusion of 250 mg as a 0.1% solution over a period of 1 hour, 125 mg/ hour for 2 hours, then 500 mcg/minute.
