Tablets: 2.5 mg glipizide and 250 mg metformin hydrochloride; 2.5 mg glipizide and 500 mg metformin; 5 mg glipizide and 500 mg metformin.

Indications & dosages
Type 2 DM with diet and exercise-Mail: Initially 2.5 mg/250 mg PO daily with a meal. Patients with fasting glucose level of 280 to 320 mg/dl, start with 2.5 mg/500 mg PO bid. May increase dose by 1 tablet daily q 2 wk. Max, 10 mg/ 1,000 mg or 10 mg/2,000 mg daily in divided doses.
Type 2 DM when diet, exercise, and sulfonylurea or metformin therapy fail - Adult: Initially, 2.5 mg/500 mg or 5 mg/ 500 mg PO bid with am and pm meals. Increase by DIABETES
