3. Indications for Use.

States of excitement, agitation and emotional tension in psychotic disorders; behavioral disorders in adults and children.

4. Action

In small doses the drug takes bland and drowsy effect, it depresses tension and agitation, it is anti-depressively active. In higher doses the drug is neuroleptic. It takes a moderate anti-psychotic and anti-vomiting action.
Anti-psychotic action develops step by step (up to several weeks); the time of effect is individual.

5. Posology and Administration

In the use of the drug in mental medicine a severity of symptoms and signs indicate a dose rate. In mild impairments it is administered 30-75 mg/daily; in medio-severe impairments it is administered 50-200 mg/daily; in schizophrenia, mania, and toxic psychosis or agitation depressions it is administered 250-800 mg/daily in in-patient clinics and 150-400 mg/daily in out-patient hospitals.
In general medicine practice in cases of mild impairments it is recommended 10 mg 3 times daily, in the state of moderate severity it is recommended 50-200 mg/daily.
Elderly patients usually are administered 25 mg 3 times a day. Dosage regimen should be changed individually.
In children an average dose of the drug is 30 mg/daily divided into 3 intakes. It may be used from 20 to 40 mg/daily.

6. Counterindications

— hypersensitivity to Mellaril and to phenothiazines;
— comatose states of whatever genesis, acute SNC;
— blood diseases;
— severe cardio-vascular diseases;
— liver diseases;

7. Should Be Taken with Precautions, if…

The drug should be administered with precaution to patients suffering from cardio-vascular diseases, closed-angle glaucoma, and prostatic hypertrophy and in possibility of epileptic attack.

When simultaneous use of Mellaril with other medications you need to take opinion of your doctor.

9. Overdosing

Drowsiness, mental fog, disorientation (down to coma and including such events as absence of reflexes, signs of paralyses), dry mouth, blurring vision, stuffy nose, orthostatic hypotonic, respiratory depression, motional disinhibition, enhancement of reflex action of segmental apparatus (spinal cord, brain stem), convulsions, hypothermia. One should not exclude the possibility of progression of acute extrapyramidal symptoms, which include disorder of nervous regulation of the work of internal parts of the body and gaze fixation when an upward glance.

10. Side Effects

On the part of nervous system, psychics and sense organs: excessive sedative effect, disorders of psychomotor functions, paradoxical effect (agitation, excitement, sleep disorders, hallucinations, enhancement of psychotic symptoms, mental fog); extrapyramidal disorders, including Parkinsonism (with motor inhibition, masklike face, tremor, salivation); dystonic reactions (tics, twisted neck); disorders of emotional reactions; disorders of the body’s temperature, drop of the threshold of readiness for convulsions. The emergence of faintness fits, dry nose, photophobia, blurred vision, myosis is also possible. Very seldom in the patients who were receiving 800 mg of the drug for a long time there is reported an emergence of pigmentation of retina, accompanying with a reduction of vision, disorders of color vision and reduction of night vision.
On the part of intestinal tract and liver: lack of appetite, elevated appetite, constipation, nausea, vomiting, diarrhea, yellow sickness, reduction of admission of bile into dodecadactylon.
On the part of endocrine status: disorders of libido, irregular menstruations, false-positive pregnancy tests, suppression of ejaculation, increase of lacteal glands and excretions from them both in men and in women, increase of weight.
On the part of cardio-vascular system: arterial hypotonia, tachycardia.
Disorders of blood formation and blood composition.
Allergic reactions: skin rash, angioedema, bronchial asthma, skin eruptions of various manifestations, contact dermatitis.
Others: swell of parotid glands.

11. Pregnancy and Lactation

It is not recommended administration of the drug during pregnancy and lactation.
