Ethinyl estradiol is a synthetic estrogen. It is a commonly used in oral contraceptives in doses of 20-50 mcg/d in combination with various progestins. Ethinyl estradiol is also given to treat menopausal symptoms and menstrual disorders in oral doses of 20-50 mcg/d. Oral doses up to 150 mcg/d are used in estrogen replacement therapy and a dose of 3000 mcg/d (3 mg/d) is used in palliative treatment of breast cancer. It is used at a daily dose of 200 mcg for postcoital contraception.

This rating is for maternal treatment with ethinyl estradiol in doses used for contraception or estrogen replacement. The risk related to very high dose treatment, as is used in some women with breast cancer is unknown, but such treatment is generally limited to post-menopausal women.

The frequency of congenital anomalies was not significantly increased among the infants of 89 women who took ethinyl estradiol during the first four lunar months of pregnancy or of 98 women who took this drag anytime during pregnancy in one epidemiological study. In another study, use during pregnancy of ethinyl estradiol was no more frequent than expected among the mothers of 171 infants with congenital anomalies. Similarly, the frequency of hormonal pregnancy tests using ethinyl estradiol during the second month of pregnancy was not increased among the mothers of 194 infants with major malformations or 551 infants with minor anomalies.
Studies in monkeys, mice, rats, and rabbits suggest that treatment of pregnant women with ethinyl estradiol in doses used for contraception or therapy of estrogen deficiency is unlikely to increase the children's risk of non-genital malformations greatly. Embryonic loss and genital alterations were observed at higher doses in cynomolgus monkeys, but such doses were also toxic to the mothers. Pregnancy loss occurred in rhesus monkeys treated with this drag at 100 times the human contraceptive dose. Pregnancy loss was frequent at even higher doses in rodents. Alterations of gonadal and internal genital tract morphology were observed among the offspring of mice treated during pregnancy with 20-2000 times the human contraceptive dose of ethinyl estradiol. Cryptorchidism was found with increased frequency among the offspring of a genetically-predisposed strain of mice treated during pregnancy with 200 times the human contraceptive dose of ethinyl estradiol. The relevance of these observations to the use of ethinyl estradiol in human pregnancy is uncertain.

Risk Related to Breast-feeding
Ethinyl estradiol is excreted in the breast milk. The amount of ethinyl estradiol that the nursing infant would receive is between 2-14% of the lowest weight-adjusted dose used for oral contraception, based on data from eight lactating mothers who took oral contraceptives containing ethinyl estradiol. This amount is similar to breast milk concentrations of natural estradiol measured in mothers not taking oral contraceptives.
No effect of maternal use of oral contraceptives containing ethinyl estradiol was observed on the physical or mental development of 48 breast-fed children followed until eight years of.
Inhibition of lactation by estrogens is dose-related so that oral contraceptives containing low doses of estrogen have little, if any, effect on milk production in well-nourished mothers.
The American Academy of Pediatrics regards combination oral contraceptives to be safe to use while breast-feeding.
