Major Uses: Amlodipine is mainly used for the long-term treatment of raised blood pressure. In addition, it can be used in the treatment of angina.
How it Works: Calcium channel blockers are thought to work by preventing the influx of calcium ions into the cells of smooth muscle which line the walls of small arteries. This decreases blood pressure and also increases the flow of blood through arteries which may be partly blocked. It may also have some action in preventing spasm of the walls of partially blocked arteries.
Usual Dosage: 2.5mg to 5mg once per day is the usual dose, though this may be increased to l0 mg.
Common Side-Effects: Swelling of the feet, dizziness, flushing and palpitations are the commonest side-effects and these are related to the dose of the drug. These are commoner at the start of treatment. Many other side effects have been reported, but they are less likely.
Actions with other Drugs: Amlodipine enhances the effects of other medications which reduce blood pressure. However, there are no reports of other significant interactions with drugs. Interestingly, the level of amlodipine in the blood is increased when taken with grapefruit juice. This is thought to be due to a component unique to grapefruit, as it does not occur with other citrus fruits.
Pregnancy, Breastfeeding: Amlodipine should not be used in women who are pregnant or attempting to fall pregnant, as theoretically it has the potential to cause birth defects. It is generally not recommended in breast feeding.
Special Features, Comments: This medication should be used with caution in people with heart failure as it may make the problem worse. Alcohol may accentuate the blood pressure lowering effects of the drug.

HTN - Adult: For patient not taking diuretics, 10 mg by mouth daily. Adjust to 20-40 mg daily in 1-2 doses. For patient taking diuretics, 5 mg by mouth daily.
