Diltiazem is a calcium channel-blocking agent used in the management of hypertension, angina pectoris, and cardiac arrhythmias. Diltiazem is given orally or intravenously in doses of 90-540 mg/d.

No epidemiological studies of congenital anomalies among infants born to women treated with diltiazem during pregnancy have been reported.
An increased frequency of embryonic loss was observed among pregnant rabbits, mice, and rats treated with diltiazem in doses respectively 2, 2, and 6 or more times those used in humans. Malformations of the limbs and tail were observed in the offspring of rabbits and rats, but not mice, treated with similar doses. No significant teratogenic effect was observed among the offspring of animals of these three species treated with smaller doses of diltiazem during pregnancy. In another study, the frequency of fetal anomalies, primarily palatal defects, and hydrocephalus, was increased among the offspring of pregnant mice treated with diltiazem in doses similar to those used in humans. The relevance, if any, of these findings to the therapeutic use of diltiazem in human pregnancy is unknown.
Diltiazem, in doses 7 times those used clinically, inhibits labor in rats at term. This effect has not been studied in humans.

Risk Related to Breast-feeding
Diltiazem is excreted into breast milk in amounts similar to those found in maternal serum. The amount of diltiazem that the nursing infant would be expected to ingest is approximately 1% of the lowest weight-adjusted therapeutic dose, based on data from a single patient.
