DIPYRIDAMOLE
(Persantine, Pyridamole)
Dipyridamole is an antithrombotic agent. It has been used in pregnancy to prevent fetal growth retardation and fetal death. It is also employed in the long-term management of angina pectoris. Dipyridamole is given orally or intravenously in doses of 60-400 mg/d.

Therapeutic doses of dipyridamole are unlikely to pose a substantial teratogenic risk, but the data are insufficient to state that there is no risk.
Maternal therapy with dipyridamole and aspirin may prevent recurrence of fetal growth retardation and fetal death in women who have had previously affected pregnancies. No increase in congenital anomalies or other adverse effects among the infants was observed in controlled trials of 119, 48, and 30 high-risk pregnancies in women who were treated with dipyridamole and aspirin in the second and third trimesters. Although a variety of congenital anomalies have been observed in children whose mothers were treated with dipyridamole during pregnancy, no consistent pattern of malformations is apparent, and the frequency of anomalies in these series does not seem unusually high.
Fetal loss is frequent among women with artificial heart valves who are treated during pregnancy with warfarin and may be even higher in those treated with both warfarin and dipyridamole. The clinical significance of this observation is unclear.
Unpublished studies in mice, rats, and rabbits are said to suggest that treatment of pregnant women with dipyridamole in usual therapeutic doses is unlikely to increase the children's risk 0f malformations greatly.
Risk Related to Breast-feeding
Dipyridamole is excreted into breast milk, al-though this has not been quantitated in published reports.
