PHYSICAL ACTIVITY AND METABOLIC DISEASES: PRIMARY PREVENTION OF TYPE 2 DIABETES MELLITUS
The CDC (Centers for Disease Control and Prevention) asserts that type 2 diabetes is largely preventable by changes in lifestyle. The director of the CDC wrote: "Restoring physical activity to our daily routines is crucial to the future reduction of diabetes and obesity in the U.S. population." The adverse effects of eliminating physical activity and increasing sedentary time (such as TV viewing) are dramatic. Certain and Kahn's study of TV viewing in childhood suggests that:
- it initiates biological and behavioral events that facilitate the long pre-clinical period before overt clinical symptoms of type 2 diabetes appear
- it substitutes sedentary time for physically active time.
Hu et al examined the relationship between TV viewing and diabetes in adults: 37,918 health professionals (physicians, dentists, optometrists, pharmacists, podiatrists, osteopaths, and veterinarians) aged 40-75 yrs reported their TV viewing for 10 years, and the data was normalized to 1.0 for the lowest level of weekly physical activity (0-5.9 MET hr/week). The relative risk of type 2 diabetes was dramatically reduced by half in those with > 40.9 MET hr/wk, compared to those engaging in the least physical activity and most TV viewing time. Every additional18 MET hr/wk of physical activity was associated with a 19% reduction in riskoftype2 diabetes. Every 2-hour increment spent watching TV was associated with an additional 50% increase in the risk of diabetes. Those viewing TV the most were at more than twice the risk.
