Tramadol Hydrochloride
It is used for strong acute and chronic pains in after-surgery period, for traumas, in oncologic patients and in other pre-surgery patients, painful diagnostic interventions, acute myocardial infarction, therapeutic procedures, and neuralgia.
It is s a centrally performing narcotic analgetic.
Adults. Usually a starting dose is 1 tablet or capsule (50 mg ) orally with a small quantity of liquid regardless of the time of feeding; if during 30-60 minutes there is no performance it is possible to take one tablet or capsule more; if the pains are severe a single dose may be 100 mg at once (2 tablets or capsule). The performance depending on the grade of frankness of pains retains during 4-8 hours. Do not exceed a day’s dose of tramadol which is 400 mg (8 tablets or capsules). Children. Children with the weight less than 25 kilos and under 14 years of age should not take this medication. Swallow tablets without chewing and rinse them with a necessary quantity of liquid, regardless of meals intake or dissolve them in a half of glass of water.
Excessive sensation to the drug. The states accompanied with breath suppression or frank suppression of central nervous system (poisoning with alcohol, sleep-inducing remedies, narcotic analgetics, psychotropic remedies). Simultaneous use of MAO inhibitors (and two weeks after their cancellation). Pregnancy and lactation period (the use is possible only on life saving indications). Age under 14 years old.
Care should be taken in patients with medicinal dependence on opioids, with a shock, unexplained confused mental state, breathing trouble, elevated intracranial pressure. To the patients who tend to misapply medicines or have dependence on them the drug is administered only if there are direct indications, short time and it is used under constant medical monitoring.
Potentiate tranquilizers, somnifacients, sedatives and narcosis remedies, alcohol. Analeptics and psychostimulants decrease the activity of Ultram, whereas naloxone and naltrexone (direct atagonists) completely block it. Together with neuroleptics it may provoke convulsions
Symptoms: coma, convulsions, myosis, vomiting, cardio-vascular collapse, decrease of AT, tachycardia, narrowing or dilatation of pupils, respiratory distress up to its stop. Treatment: emergency measures – aspiration, breathing and cardio-vascular support.
Nausea, vomiting, dizziness, failure of attention, perspiration. In therapeutic doses it does not provoke a frank respiratory distress and does not considerably affect blood circulation and intestines.
Ultram penetrates through placental barrier. During pregnancy it is better to avoid a long-term use as it may result in a cancellation syndrome in a neonatal period. If Ultram is used before or during birth, the drug does not influence on the constriction of womb. It may induce some decrease of frequency of breathing in newborns which clinically does not play a considerable role. During lactation nearly 0,1 % of the concentration of Tramadol in plasma excretes though milk. After a single intake of Tramadol usually there is no necessity to interrupt breastfeeding
Driver of vehicles and people whose work is connected with alertness are not recommended to use Tramadol.
